How Do I Select the Right Skilled Nursing Facility?

Once you acquire the names of several Skilled Nursing Facilities, you will want to learn more about their services and quality of care.  The following checklist provides questions to help you determine which provider is best for you or your loved one.

*Note: this list is not all inclusive and is provided as a guide for consumers.

  1. How long has this provider been serving the community?
  2. Does this provider supply literature explaining its services, eligibility requirements, fees, and funding sources? Many providers furnish patients with a detailed "Patient Bill of Rights" that outlines the rights and responsibilities of the providers, patients, and caregivers alike. An annual report and other educational materials also can provide helpful information about the provider.
  3. How does this provider select and train its employees? Does it complete background checks? Does it protect its workers with written personnel policies, benefits packages, workers’ compensation and malpractice insurance?
  4. Do nurses or therapists consult the patient's physicians and family members and include the patient and his or her family members in developing the plan of care? Are they involved in making care plan changes?
  5. Does this provider take time to educate family members on the care being administered to the patient?
  6. Who can the patient and his or her family members call with questions or concerns? How does the facility follow up on and resolve problems?
  7. What are the financial procedures of this provider? Does the provider furnish written statements explaining all of the costs and payment plan options associated with care?
  8. What procedures does this provider have in place to handle emergencies?
  9. How does this provider ensure patient confidentiality?
  10. Request a tour of the facility. Consider whether or not the facility provides tours only at designated intervals, or if they are comfortable touring visitors without an appointment.  If you tour during a meal, are patients clean, dressed, and being assisted? Are the staff engaged with the patient, or are the patients being ignored? Does the food look appetizing?
  11. Talk to other visitors or patients to inquire about their impressions of the facility. Observe the facility for: odors, cleanliness, cheerfulness, comfort, home-like environment. Do patients have personal items in their rooms? Are they dressed in their own personal clothing?
  12. Inquire about visiting hours. California State regulations require open visiting in the skilled nursing facility, 24/7.
