
What is the H.E.L.P. Alliance?
The H.E.L.P. Alliance is a group effort among leadership-level community members from area hospitals, non-acute medical settings, social and public services who are motivated and committed to improving collaboration along the health care continuum through education, training and a multidisciplinary approach.
The H.E.L.P. Alliance’s purpose is to improve collaboration among service providers and public safety organizations by:
Improving the coordination of care across the care continuum for high risk seniors who transition between care settings, such as hospital to skilled nursing facility or home, or home to the emergency room.
Collaborating as a community to identify and resolve barriers to care in the non-acute care setting.
Increasing access to resources within the community of this vulnerable population.
Why?
The H.E.L.P. Alliance was formed to address community issues, such as:
Older patients with chronic illnesses who often require care from a variety of practitioners in multiple settings, yet many settings are disconnected from others, creating potential quality gaps.
State cuts that have resulted in fewer resources to help the frail, elderly senior population remain independent at home
Increased numbers of isolated seniors below the poverty line who do not have financial resources to pay for services
Lack of or limited access to transportation for seniors in our community
Inadequate home-based support services and delays in needed health care that result in unnecessary “revolving door” use of emergency/urgent care services
What population does the H.E.L.P. Alliance serve?
Seniors living in Southwest Riverside County and the surrounding area.
Meetings
The members of the H.E.L.P. Alliance meet monthly to:
Receive education on new or innovative community programs and/or services.
Collaborate on individual cases to identify alternative resources to meet client needs.
Brainstorm solutions to community issues.
To maintain privacy and confidentiality in accordance with state and federal laws, membership in H.E.L.P. Alliance is by invitation of the Alliance Executive Committee, and approval by members. However, community needs and issues may be submitted to any of our members for discussion at the Alliance.
